In order to succeed
in today’s health care environment, Interprofessional
teams are essential. Interdisciplinary or Interprofessional
is not just random professionals speaking to each other. This model is
inclusive to different to better the patient’s health. Interdisciplinary team
work improves the diagnosis and prognostic abilities of health. Team work is
critically important to provide safe and effective care. This collaborative
learning aims redesign care delivery to provide optimal value to hospitalised
patients. The use of multidisciplinary in hospital team improve health outcomes
and reduces adverse events e.g. mortality.
A team is defined as two or
more individuals with specified roles and dynamic towards a shared goal and
better patient health outcomes. The ultimate goal of interdisciplinary team is
to improve patient outcomes by doing interdisciplinary rounds within the
hospital, this provides a forum for regular interdisciplinary communication,
setting the foundation to better health outcomes of hospitalised health
outcomes. This team also improve health outcomes by communicating with
patients, communities and other health professionals in a responsive and
responsible manner that supports a team approach to the maintenance of health
and the treatment of disease.
In society we need to consider
the outcomes for people living with HIV. Interprofessionals
will refocus their efforts to consider the health and social outcomes that
require attention from the whole healthcare community. Their goal is to is to
take comprehensive view to the care management and treatment of people living
with HIV. Interprofessional will
improve long term health and social participation and also will look at the
impact of HIV and anti-retroviral therapy. Interprofessional
use the application of implementation science with HIV, this has a positive
impact on the quality of life and wellbeing of people living with HIV. Interprofessional improves the patient
and healthcare worker satisfaction and it decrease length of hospital stay. Interdisciplinary
optimise medication dosage and reduces preventable adverse drug reaction.
There are many
benefits to Interprofessional collaboration(IPC),
it can improve safety and healthcare delivery as well as reduce cost. It also
put patient at the centre of the health care team’s focus and allow all health
professionals, with the patient, to collaboratively provide input and improve
outcomes. Interprofessional
collaboration holds promises for reducing medical errors, improving the quality
of care and meeting the needs of diverse populations. Interprofessional perform effectively in various team roles to
deliver patient-or population-centred care that is safe, timely, effective and
equitable.
